Terms applicable to promotional offers, free trials, and discounts made available by CaliberRx.

1. Scope
These Offer Terms govern specific promotional, trial, and discount offers made available by CaliberRx LLC. They
supplement and are incorporated into our Terms of Service, Payment & Billing Consent, and Refund Policy. Where an
individual offer has its own specific terms presented at checkout, those specific terms control over any conflicting
language in these Offer Terms.
2. Eligibility
- Offers are available only to eligible new or existing members in states where the Provider Group is licensed and as further specified in the offer.
- Unless stated otherwise, each offer is limited to one redemption per person, household, payment method, or email address.
- Offers are not transferable, are not redeemable for cash, and cannot be combined with other offers unless expressly permitted.
- CaliberRx may require proof of eligibility and may rescind an offer if eligibility criteria are not met or if the offer has been redeemed fraudulently.
- Government insurance. Promotional pricing, free trials, discounts, and similar offers are not available to individuals enrolled in Medicare, Medicaid, TRICARE, MediCal, the U.S. Department of Veterans Affairs health benefits program, the Indian Health Service, or any similar federal or state healthcare program. This restriction is intended to comply with the federal Anti-Kickback Statute (42 U.S.C. §1320a-7b) and analogous state laws. By redeeming an offer, you represent that you are not enrolled in any such program for the services covered by the offer.
3. Free Trial Offers
Where a CaliberRx offer includes a free trial period, you will not be charged the applicable membership fee during the
trial. A valid payment method is required at enrollment and will be used to process any charges for non-trial items (such
as consultation fees or medication) during the trial. If you do not cancel before the trial ends, your membership converts
to a paid subscription at the price then in effect and your payment method is charged automatically. You may cancel at
any time through your account portal or by emailing support@caliberrx.co.
4. Promotional Pricing
Discounted pricing is honored for the duration expressly stated in the offer. When the promotional period ends, your
membership renews at the then-current standard rate unless you cancel. We will provide advance notice of the renewal
price as required by our Payment & Billing Consent and applicable law.
5. Gifts and Add-Ons
Where an offer includes a gift or add-on (such as a supplement, accessory, or bonus service), the gift is provided on an
“as available” basis and may be substituted with an item of equal or greater value. Gifts have no cash value.
6. Modification or Termination of Offers
CaliberRx may modify or terminate any offer at any time without notice, except as required by law, provided that any offer
you have already accepted will be honored in accordance with its terms as presented at the time of acceptance.
7. Refunds on Promotional Purchases
Our Refund Policy applies to promotional purchases. Amounts paid under a promotional price may be refunded only as
permitted by the Refund Policy and subject to any additional conditions specific to the offer.
